Causes

There are certain predisposing or associated factors that influence the incidence and development of UTI. These factors include:

Age & Sex: UTI is common in women of all ages. Factors responsible for this could be the short length of the female urethra, and sexual activity. UTI is uncommon in men below the age of 50 years.

Pregnancy: Both symptomatic and asymptomatic urinary infections are common during pregnancy. Relaxation of the urinary tract could be a predisposing factor.

Diabetes Mellitus: There is a high incidence of urinary infections among diabetics. The presence of glucose in the urine, itching in the vulva, neurological involvement of the bladder and vascular changes favor the development of infection.

Urinary Tract Obstruction: Obstruction of the urinary stream at any level within the urinary tract is the most common predisposing factor for infection.

Use of Instruments: Catheterization or the use of other instruments may result in the introduction of bacteria into the urinary system.

Some other factors leading to urinary infection include, controlling the urge to pass urine for long hours, inadequate consumption of water and excessive intake of acidic food.

Signs and Symptoms

More than 50% of patients with UTI are asymptomatic.

A symptomatic patient with urinary tract infection generally has fever, chills, frequent urination and pain in the flanks.

When the infection is limited to or is predominantly in the lower urinary-tract, frequent urination, burning sensation, suprapubic heaviness or dysuria (pain while passing urine) are the common symptoms.

Urine may be turbid, and occasionally, gross haematuria may be present. Severe colicky pain may be present because of an associated calculus.

In children, gastrointestinal symptoms like loss of appetite, nausea, vomiting, diarrhea or abdominal pain may be the predominant features.

On physical examination, the only signs of UTI may be evidence of flank or renal angle tenderness and some tenderness over the suprapubic region. Children may show signs of dehydration and toxemia.

If not treated in time, UTIs can lead to scarring in the kidney, a tendency for the formation of stones or septicemia. In rare cases, they can progress into acute renal failure.

Diagnosis

Diagnosis can be made on the basis of the following tests:

Urine: Urine may have a turbid appearance, on examination by the naked eye. Protein is present in traces. Microscopic examination reveals the presence of white blood cells. Red blood cells may also be present.

Blood: In an acute stage, total white-blood-cell count is raised. It is good practice to assess the basic renal function by estimating blood urea and creatinine.

Radiology: A plain X-ray of the abdomen, an intravenous pyelogram and a micturating cystogram are useful in excluding predisposing factors.

Treatment

General Measures: During the acute stage, rest, adequate fluid-intake and analgesics are helpful. Excessive fluid intake is recommended with the aim of flushing out bacteria. Alkalis may also help to reduce irritation in the lower urinary-tract.
