Acute Pancreatitis

Acute Pancreatitis is an acute inflammation of the pancreas, typically presenting with abdominal pain and raised pancreatic enzymes. It has an incidence of 5-10 per 100,000 people.

Causes

The pancreas secretes digestive enzymes, which are activated in the intestine. Acute pancreatitis occurs when these enzymes are activated in the pancreatic duct system. Risk factors for the same are as follows:

Signs and Symptoms

Pain - The onset is sudden, with severe pain in the epigastrium or right side of the abdomen. It often occurs within 12-24 hours of a large meal or alcohol. Pain is usually persistent, and most frequently radiates through the back, to either shoulder or to one of the iliac fossae, before spreading to involve the whole abdomen.

Nausea and vomiting are common.

In severe cases, shock supervenes, and there is increased heart rate, low blood pressure and kidney failure.

On examination, there may be fever, jaundice, and bluish discoloration of the flanks or around the umbilicus.

Sometimes, abdominal rigidity, ascites and breathlessness are seen.

Complications

Abscess formation; hemorrhage in the pancreas; vomiting of blood; diabetes mellitus.

Diagnosis

Blood Tests - Serum Amylase is elevated on the first day of the disease. A five-fold rise is diagnostic of the disease.

Serum Lipase is elevated for longer periods than Serum Amylase.

Blood Sugar may reveal a transient rise.

Serum Calcium levels may fall, which may lead to tetany.

Fibrinogen level falls especially in hemorrhagic pancreatitis.

Urine Tests - Urinary amylase activity measured on a 24 hour collection of urine may be helpful when the serum level is not diagnostic.

X-rays - These are helpful in detecting the cause of pancreatitis.

Imaging - Ultrasound and CT scan are used to confirm the diagnosis, and are vital in monitoring the progression of the disorder.

Treatment

Fluids are essential and large quantities are required to replenish those lost. Hence, adequate quantities of glucose, saline, albumin, plasma and blood may be required.

Pain is best relieved by intravenous or intra muscular administration of Pethidine; Morphine is contraindicated.

Suppression of pancreatic secretions is done by continuous gastric tube aspiration. Oral antacids every 3 hours decrease gastric acidity and decrease pancreatic stimulation.

Antibiotics are used if there is evidence of infection.

Avoiding causative factors helps prevent relapses.

Surgery is necessary when there is an abscess or when there is inflammation of the gall bladder.

Chronic Pancreatitis

Chronic Pancreatitis is defined as a continuing inflammatory disease of the pancreas, characterized by irreversible morphological changes and typically causing pain and/or permanent impairment of function.

Signs and Symptoms

The disease is most common in men between the ages of 35 and 45.

Nearly all patients present with abdominal pain. Recurrent attacks occur at intervals of several weeks or months, often within a few hours or two days of an alcoholic bout.

Pain begins gradually and persists for days or weeks. Pain is located in the epigastrium, right or left sub costal areas or around the umbilicus. Relief may be obtained by crouching forward or leaning forward over a chair.

Weight loss is common due to malnutrition, secondary to pancreatic pain and diabetes mellitus.

Diarrhea is common.

Diabetes mellitus develops in about a fifth of patients.

Steatorrhea (fats in stools) develops in about one third of patients, and may occasionally be the presenting feature.

Jaundice may develop due to obstruction of the common bile duct.

On examination, there may be diffuse tenderness in the upper abdomen and features of malnutrition and weight loss.

Diagnosis

A variety of investigations may be used to confirm the diagnosis of Chronic Pancreatitis. They may be classified into Radiographic and Imaging techniques, which demonstrate the structural changes in the gland and its function, and Metabolic Studies, which indicate whether the function of the gland is inadequate for normal physiological processes.

Stool Test - A 5-day stool collection test to demonstrate steatorrhea (fats in stools).

Liver Function Tests - Raised Serum Alkaline Phosphatase may indicate obstruction of the bile duct.

Glucose Tolerance Test, to demonstrate diabetes mellitus.

Plain X-ray of the abdomen, to detect calcification of the pancreas.

Ultrasound and CT Scan, to demonstrate atrophy and calcification, temporary enlargement in acute attacks, and stricture of common bile duct.
