Symptoms

Most patients with Osteoporosis are relatively asymptomatic until they develop a complicating fracture, which often occurs after minimal trauma. Fractures are most common in the hip, humerus, ribs and wrists.

The most common symptoms are pain in the back, deformity of the spine and loss of height. Pain usually results from collapse of the vertebral bodies, especially in the lower dorsal and upper lumbar regions. It is typically acute in onset. Such episodes frequently occur after sudden bending, lifting or jumping movements, which may be trivial.

Some people may only have asymptomatic vertebral fractures.

Types

There are two main categories of Osteoporosis:

Type I Osteoporosis occurs only in post-menopausal women, and is due to estrogen deficiency.

Type II Osteoporosis occurs in both men and women, but, about twice as frequently in women. It occurs due to aging, and calcium deficiency.

Causes

. Calcium and/or Vitamin D deficiency from malnutrition increases the risk of osteoporosis.

. Estrogen deficiency following menopause is correlated with a rapid reduction in BMD (Bone Mass Density).

. Hormone deficiency states such as testosterone deficiency can lead to osteoporosis.

. Glucocorticoid or thyroxine excess states also lead to osteoporosis.

. In pregnant and lactating women, the rate of bone loss temporarily increases if the increased demands of calcium are not met by adequate intake.

. Other significant factors leading to the onset of osteoporosis include hypothyroidism, anticonvulsant medications, a sedentary lifestyle, smoking, inadequate physical activity (weight bearing exercise), and family history.

Diagnosis

. Osteoporosis is most commonly discovered either on routine examination, or following a pathologic fracture (a fracture due to weakened bone).

. Densiometry or DEXA scan is the most useful screening test to detect osteoporosis. While this test does require special equipment, it is safe. It exposes the patient to small amounts of radiation, and is useful in detecting early stages of osteoporosis.

. X-rays usually show a generalized loss of bone density.

. Laboratory studies are not very helpful in diagnosing osteoporosis, but they are helpful in showing evidence of conditions that can have symptoms similar to osteoporosis. Laboratory studies can show evidence of osteomalacia, kidney failure, parathyroid gland insufficiency, or other problems that may mimic osteoporosis and cause bone weakness.

Treatment

The primary goal of treatment of osteoporosis is to reduce the risk of pathologic fractures.

The three mainstays of treatment are:

1) Weight-bearing exercises:

Weight-bearing exercises and movements are prescribed for osteoporosis. Exercise cannot replace bone mass that has already been lost, but it can strengthen the bones. Simple movements can bring flexibility and agility to the joints. Agility helps to maintain balance and prevent falls.

While resistance exercise (e.g. with light weights) are excellent to stimulate bone production, even light aerobic exercises (e.g. aerobics, jogging, walking) can help strengthen bones and prevent osteoporosis.

2) Nutritional supplementation:

Calcium supplementation is an important treatment. The daily recommended intake should be at least 1200 to 1500 mg per day. Obtaining this amount of calcium through natural sources is difficult, considering that it is equivalent to drinking 5 glasses of milk each day. Several different medications are used for treating osteoporosis. These include:

Bisphosphanates are a type of medication that help regulate calcium retention and prevent bone resorption.

Calcitonin is a naturally occurring hormone produced by the thyroid gland. It can be given in an injection or in the form of a nasal spray. It inhibits the function of osteoclasts - the cells that break down bone.
