What are Hemorrhoids?

The anus and lower rectum are very rich in blood supply.

The rectum is at the end of the colon, where feaces gets collected before it is expelled through the anus, as a bowel movement. When the blood vessels in this area are stretched under pressure, they swell up and form lumps. These lumps are called hemorrhoids. They can be internal (patient cannot see them) or external (can be seen hanging out from anus). A person may have both internal and external piles at the same time.

Internal hemorrhoids are classified further, based upon the degree to which they protrude from the anal canal.

According to this grading system:

--Grade I hemorrhoids may bulge into the anal canal but do not protrude through the anus.
--Grade II hemorrhoids protrude through the anus during straining and defecation, but return spontaneously.
--Grade III hemorrhoids protrude through the anus while defecation or straining but do not return spontaneously, requiring the patient to gently push it back into its normal position with a finger.
--Grade IV hemorrhoids cannot be manually returned to their normal position.

Causes

The precise reason why piles occur is not known, but it may be linked with excess pressure in the anus and the lower rectum. It develops when the veins are under constant pressure and get engorged with blood. If the pressure continues, the walls of the veins are stretched. Such a distended vessel leads to fluid accumulation and swelling in the surrounding tissue, leading to hemorrhoid formation. Some expert studies suggest that hemorrhoids are caused by continuous high pressure in the veins of the body, which occurs because humans stand upright. It can occur at any age.

One may have an increased risk of getting piles in the following conditions:

Symptoms

Diagnosis

The diagnosis of Hemorrhoids (piles) is based on medical history, physical examination and visual inspection of the anal canal and rectum.

External hemorrhoids can be confirmed clinically with a visual examination followed by a rectal examination. However, diagnosis of internal hemorrhoids may sometimes be difficult during rectal examination. In such cases, an instrument called an Anoscope is inserted into the anal canal to view the lining. This is an OPD procedure. A more detailed observation can be carried out by a procedure called Sigmoidoscopy that is done under sedation or anesthesia. Besides these, the physician may also advise for stool culture, to detect the presence of occult blood.
